Rates & Insurance

For patients who are paying out-of-pocket (not with insurance), rates are as follows:

*First-time treatment (80 minutes) $100
*Follow-up treatments (60 minutes) $85
*These fees reflect a time-of-service discount which is not applicable with insurance billing.

All out-of-pocket payment, copayments, or coinsurance are due at the time of treatment.

Cash, check, credit cards, and flex spending cards are accepted.

I am a preferred provider for the following insurance companies and will gladly bill them as a convenience to you if acupuncture is included in your plan.

  • *Any auto insurance company
  • Aetna
  • American Specialty Health (ASHN)
  • Blue Cross Blue Shield
  • Blue Cross Blue Shield Federal Employee
  • Cigna
  • CHP (Complementary Health Plans)
  • Health Net
  • Kaiser Permanente
  • LifeWise
  • Moda (formerly ODS)
  • Optum/United Health
  • Pacific Source
  • Providence
  • Regence Blue Cross

I am not currently accepting Worker’s Compensation claims of any kind.
I do accept any other insurance that covers acupuncture treatment from out-of-network providers.

Please know that with some insurance, your deductible does not apply to acupuncture. In that case, your acupuncture will be covered right away, before your deductible is met for the year. With other insurance, your deductible will need to be met before your acupuncture coverage kicks in. In those cases, you will need to pay for your treatment until your deductible is met. Those payments will be submitted to your insurance company to be applied toward your deductible.

*Auto Accidents: If you have been in a car accident in Oregon and your treatment is in regards to injuries sustained in that accident, you do not need a referral from another provider to seek acupuncture. You are almost always covered 100% provided that the accident has been within the last year and you have not exhausted the claim’s medical benefits.

To verify your coverage, please check your benefit information or call your insurance company with the following questions:

Do I have acupuncture coverage?
If so, what are the limits (either a dollar amount or a number of visits allowed annually)?

Do I need prior authorization or a referral for acupuncture? (If yes, please check in with Susan about how to get that.)

Is Susan Wilmoth, LAc a provider in my network?
If not, do I have coverage from out of network providers? (In that case, do I have a separate deductible for out-of-network care? If so, how much?)

What is my copay or coinsurance?

Do I have a deductible that applies to acupuncture?
If so, what is my deductible?
How much of that has been met at this time?

Do I have an out of pocket max?
If so, how much?
How much of that has been met at this time?

Is my insurance on a calendar year? If not, what is my “plan year”?
